The bathrooms at Milk & Honey are nice, with Kiehl's products. The bathroom at Double Happiness - very peaceful if you're feeling pukey. And the ladies' room at Union Square Cafe is way too small to have two stalls.
I'm a born and bred New Yorker but I'd encourage all of us not to get too myopic.

The HK Felix's bathroom are indeed amazing. My aunt was so impressed with her side that she had to snap a picture.
In Japan, ALL bathrooms are decadent. It takes a while to get used to electric toilet seats, but they are very nice in the winter.
i agree with eli, having used japanese toliets, there's something about not having to get your hands dirty. plus, the water is warm so it feels strangly good...that sounds so gay....

Gramercy Tavern in NYC has the nicest (women's) bathrooms I've used.
